Some employers offer their employees a Cafeteria Plan benefit that allows you to use pre-tax dollars for medical expenses not covered by insurance. Please check with your employer to see if this option is available where you work.
Insurance and Payment
The fee for an initial evaluation will vary, depending on the complexity of your problem.In an effort to minimize our administrative costs, and thereby our fees, we request that payment for all office services be made at the time of your visit.We are happy to accept your personal check, MasterCard, Visa and Discover.
Our professional services are rendered to you and not your insurance company.Therefore you and not your insurance company are responsible for payment of all charges.If you have an account that lingers unpaid for three months, you should be aware that we will take the necessary action to have it paid.We fully understand that there are times you omay not be in a position to pay our fees.If this should occur, please contact us so arrangements can be made to handle your account.
As a service to our patients, we will be glad to assist you in filing your primary and secondary insurance claims.Remember that in order for us to assist you with this, we must have your current, accurate insurance information.A monthly statement for any unpaid balance will be mailed to you.As a Medicare provider, we also accept assignment on all Medicare patients.
Please let us know if you are a member of a Health Maintenance Organization (HMO) or a Preferred Provider Organization (PPO).If we are a participating provider, we will submit your claims for you.Some plans require referral from a primary care physician.Please let us know if your plan requires this when making your appointment.
